Community Mental Health Services

People can experience mental health difficulties at any point in their lives and the impact on their well-being will vary between individuals. This can depend on the diagnosis they receive and how it impacts their day-to-day functioning.

 

Vale Locality Mental Health Team (VLMHT) 

Mental health services in Wales are generally set out in a piece of law called the Mental Health (Wales) Measure 2010, which sets out the support people are entitled to. Fundamentally services are divided into Primary and Secondary. Primary services are generally at GP level where individuals are experiencing mild to moderate mental health problems, or those with more serious mental health problems who are currently stable.

 

If you have concerns regarding your mental health or the mental health of a family member/friend, in the first instance you should contact your GP. Your GP can offer medication, talking therapy within the practice, signposting to other services or referral to other services depending on your needs.

 

The VLMHT comes within ‘Secondary Care’ and provides support with assessed needs. Assessments for Social Care are undertaken, provided and reviewed in line with the Social Services and Well-Being (Wales) Act 2014. This strength based approach works to promote well-being. Vale of Glamorgan Social Workers work within the Vale Locality Mental Health Team based at Barry Hospital. They work alongside Psychiatrists, Community Mental Health Nurses, Occupational Therapists, Psychology, Health care assistants and community support workers.

 

Approved Mental Health Professional (AMHP) Service: Within day-time services, the Vale of Glamorgan operates the AMHP service. This provides Approved Mental Health Professionals to carry out functions of the Mental Health Act particularly around assessment for compulsory admission under the Mental Health Act.

Community Mental Health Service for Older People 

The Vale of Glamorgan Community Mental Health Service for Older People is a multi-disciplinary, team of mental health professionals including social workers, mental health nurses and psychiatrists.

The team offers specialist secondary mental health care for adults living with dementia and people over 65years with mental health conditions.

The team works alongside specialist NHS services to foster the resilience and recovery of older individuals within our community facing mental health challenges. The team is committed to providing holistic and personalised support, ensuring that every individual feels valued, understood, and respected throughout their journey towards mental health recovery. We recognise the uniqueness of each person's path and tailor our interventions to meet their specific needs, focusing on promoting independence, well-being, and quality of life.

 

Who do we support?

  • Adults who are 65 years or over with a secondary mental health need who are open to Out patients and live in the Vale of Glamorgan.
  • Those who look after adults who are 65 and over (carers)

It is more likely you will be assessed as needing care and support if you:

 

  • Are experiencing symptoms of a severe and enduring mental health condition, such as severe depression, psychotic disorders, severe anxiety disorders, or cognitive impairment such as dementia.
  • Have a complex mental health condition that requires specialised assessment, treatment, and support beyond the scope of primary Mental Health Care Services.
  • Are experiencing distressing symptoms that significantly impact on your daily functioning, relationships, or overall quality of life

Can anyone use this service?

To access secondary mental health services a referral from your GP would be required for Out Patients.

A wide range of services are available in the Vale of Glamorgan to support people who are suffering with mental health issues. If you are concerned that you or someone you know has a mental health problem, you should first go to your local GP surgery.
